Auxiliary Scholarship

Thank you for your interest in the Boone County Hospital Auxiliary Scholarship. This scholarship has been established to provide financial assistance for tuition and books for a Boone County Hospital employee (FT, PT, PRN) pursuing an education in a health related field. The criteria for eligibility includes:
- Acceptance in an accredited college or trade school on a full-time basis
- Completion of the scholarship application
- Enrollment in a course of study related to the health field
If selected, the recipient(s) will be notified by mail and the scholarship(s) will be paid in two $500 installments, by the semester, and made payable to the recipient(s) and the institution(s). If the student drops out of the program, they must notify the hospital auxiliary of the change. Application is for one year and may be reapplied for the following year. Applications must be mailed or delivered to the attention of Scholarship Chairperson, Boone County Hospital Auxiliary, 1015 Union, Boone, IA 50036.
Information to be included with your application must include:
- Two letters of reference
- Letter of application stating school and community involvement and future goals
Auxiliary scholarship deadline is July 15th.
BOONE COUNTY HOSPITAL SCHOLARSHIP APPLICATION

- A grade point average of at least 3.0
- Acceptance in an accredited college or trade school on a full-time basis
- Completion of the scholarship application
- Enrollment in a course of study related to the health field
If selected, the recipient will be notified by mail and the scholarship will be paid in two $500 installments, by the semester, and made payable to the recipient and the institution. CID (CYNTHIA) METCALF MEMORIAL SCHOLARSHIP

This $500 scholarship has been established in memory of Cid (Cynthia) Metcalf. Cid was an employee of Boone County Hospital for thirty-two years. Her career ended as she was getting ready for work and had a stroke. Cid began her career at Boone County Hospital as an OB technician and continued to expand her knowledge and training, reaching the position of Accountant. Her attention to details earned her the respect of Administration and the financial auditors.
Cid grew up in Eagle Grove and attended in Community college in Fort Dodge. She has many family members and friends in the Eagle Grove, Fort Dodge, and Webster City areas. She was an avid fan of many sports. In the spring, she and her husband would host NCAA parties where the games were on multiple TVs, and in the fall she wouldn’t miss a Raiders football game. Cid also enjoyed being outdoors working in her yard and seeing the many different animals that would come to her feeders. Family was very important to Cid and enjoyed organizing family gatherings.
The purpose of this scholarship is to honor Cid’s memory by establishing a scholarship for a student pursuing a business or technical/professional program. It is hoped that the recipients of this scholarship will approach their career with the same commitment and joy that Cid did.
Preference will be given to students in financial need, and/or those who have, or are trying to, overcoming adversity, and/or demonstrating that they are the type of person who will bring to the profession the qualities that Cid did.
Eligibility Criteria:
- A grade point of at least 3.0
- A student in good standing at their current high school or technical school
- Completion of scholarship application

LILA CARPENTER MEMORIAL NURSING SCHOLARSHIP APPLICATION

This scholarship has been established in memory of Lila Carpenter. Lila was an employee of Boone County Hospital for thirty-one years. Lila overcame many adversities during her lifetime. With the encouragement of a physician, she returned to school and became a Registered Nurse.
Lila approached all of her patients in a very professional and caring manner. Her positive attitude was contagious. She had the ability to make her patients feel like getting better. Lila understood the immense power of a positive attitude and the ability to genuinely care for, and about, another person.
Lila enjoyed life and “never gave up”. She challenged all obstacles in her path and never lost her faith in God, her family and her friends.
The purpose of this scholarship is to honor Lila’s memory by helping selected nursing students complete their nursing education. It is hoped that the recipients of this scholarship will approach their nursing careers with the same commitment and joy that Lila did.
Preference will be given to students in financial need, and/or those who have, or are trying to, or have, overcome adversity, and/or demonstrate that they are the type of person who will bring to the nursing profession the qualities that Lila did.
$700 in scholarship money will be awarded to help a special student during the final year of their nursing education at the DMACC Boone Campus. Any second year student is eligible irregardless of when their second year begins or ends.
Eligibility Criteria:
- A grade point of at least 78%
- A second year nursing student in good standing at DMACC Boone Campus
- Completion of scholarship application

PUNTENNEY SCHOLARSHIP APPLICATION

This scholarship has been established in memory of Sigrid E. Puntenney and in honor of Sandra L. Puntenney.
Sigrid E. Puntenney was a resident of Boone for 63 years. She was the widow of A. Wallace Puntenney, M.D. When their son Kevin died at the age of 10, she and Wallace established a Math and Science award in his memory. This award recognizes students who excel in math and science by earning maintaining a 4.0 grade point average during their final two years in high school. At the time of Sigrid’s death in 2010, the award was in its 50th year. During those 50 years nearly 1,000 of the approximately 10,000 students graduating from the Boone High School had been recognized for their academic success by receipt of this award.
Sandra L. Puntenney is a Registered Nurse whose career spanned 43 years. She was a graduate of the Mercy School of Nursing in Des Moines, Iowa and Ottawa University of Shawnee Mission, Kansas. In July of 2010 Sandra retired after nearly 18 years as the Assistant Administrator for Nursing at the Boone County Hospital. During her tenure at the Boone County Hospital she was actively involved in developing and supporting the Tuition Assistance Program that enabled hospital employees to complete their nursing education. She was also a member of the Lila Carpenter Memorial Nursing Scholarship Committee.
The purpose of this scholarship is to honor the accomplishments of these two women by continuing to offer financial assistance to Boone County Hospital employees, or their children, as they pursue basic or advanced education in a healthcare-related field. This may include careers in Nursing, Radiology, Laboratory, Respiratory Therapy, Physical Therapy, Occupational Therapy, Speech Therapy, Pharmacy, Medicine, etc.
