Community Health Needs Assessment
Boone County Hospital and Clinics is conducting a Community Health Needs Assessment (CHNA) to evaluate the overall health needs of the area. The CHNA provides many organizational and community benefits such as identifying community health needs, health disparities, and key service delivery gaps.
We are asking community members to participate in a survey that will help us complete this assessment. The survey will run from March 17 through April 7, 2025, and your anonymous response will help us understand diverse community perspectives, prioritize the health needs in our area, and develop an action plan to address those needs.
