Appointments will be held at Boone County Hospital.  Please bring your completed Client Information Form, insurance cards, prescription list, cell phone with text/email access, and any plan-related letters or statements.

As a reminder, Medicare prescription drug plans can change the drugs they cover, preferred pharmacies, restrictions, monthly premiums, deductibles and drug costs.  It’s important for anyone using Medicare to review their drug plan coverage every year.
